Shimberg Library Passport Services: Using Shimberg Library Passport Services

Obtaining a New United States Passport

If you've never had a U.S. passport, are 16 or under, or aren't allowed to renew an existing passport by mail, you need to apply for a new passport in person. Please allow 4-6 months for this to process. Follow the steps below to obtain a new passport at the Shimberg Library.

Step 1: Complete the Application

  • Obtain a FormDS-11: Application for a U.S. Passport.
  • Complete the application (black ink only) prior to arrival. Do not sign application until instructed to do so during the acceptance process.
  • Determine which passport is right for you?
    • Passport Card - A passport card allows land and sea travel from U.S. to Canada, Mexico, Caribbean and Bermuda only.
    • Passport Book - A passport book allows any method of travel internationally to any country.

Step 2: Gather Original Citizenship and Identification Documents

  • Proof of U.S. Citizenship - Bring one* of the following items, plus a photocopy of that item:
    • Original Birth Certificate issued by the city, county, or state.
    • Previously issued, undamaged U.S. Passport.
    • Consular Report of Birth Abroad or Certification of Birth.
    • Naturalization Certificate** or Certificate of Citizenship.
  • Primary Identification - Bring one* of the following items, plus a photocopy of that item:
    • Valid Driver's License (copy both sides); if out-of-state, additional ID is required.
    • Previously issued, undamaged U.S. Passport.
    • Naturalization Certificate**
    • Current Military ID (military or dependents) or Government ID (city, state, or federal).

*Original documents will be submitted to the U.S. State Departmeny

**Naturalization Certificate also requires submission of certified Birth Certificate if applicant is under 16 years of age.

For Applicants Under 18
  • Applicants age 16 & 17: Parent or legal guardian can present Primary ID if minor applicant does not have one.
  • Applicants Under 16: Both parents listed on birth certificate/adoption papers/legal guardian court documents must be present. See Passports for Children Under 16 for more information.

Step 3: Get Your Passport Photo

You need one 2" x 2" color passport photo. See Passport Photos for requirements and examples. Passport photos are available at the Shimberg Library for $12.99, payable by credit card, Google Pay, and Apple Pay. Please arrive 10 minutes prior to your appointment time if you plan to use our photo booth to take your passport photos. Not for use by children under the age of 6.

Step 4: Bring Your Unsigned Application Form, Documents, Photo and Payment to Your Appointment

All applicants must be present, regardless of age.
Two separate fees are due at the time of application:
  1. Check or money order made payable to U.S. Department of State for passport fee:
    Passport Type Age 16+ Under 16
    Book $130 $100
    Card $30 $15
    Book + Card $160 $115
  2. Debit/credit card, ApplePay, GooglePay, or AndroidPay for $35 execution fee.
Optional Fees paid to the U.S. Department of State:
  • $60 – Expedite Fee paid per application, in addition to required fees. Provides faster processing than routine service through the U.S. Department of State.
  • $19.53 – 1-2 Day Delivery Fee paid per application for 1-2 day delivery of an issued passport book from the Department of State to the customer. Only applies to mailing addresses within the United States. Not valid for passport cards.
  • Need your passport sooner? The Shimberg Library cannot accept applications for passports needed sooner than the State Department's current expedited processing times.

How to Find Us

We are located at 12901 Bruce B. Downs Blvd. in Tampa, which is on the east side of the road, just south of the pedestrian bridge between USF and the James A Haley VA Hospital.

We have a parking space dedicated for our passport clients in the lot nearest to our building in lot 30 (see map below). If the dedicated space is in use, there are also two metered spaces in this lot, payable with debit or credit card. The library is the building located directly north of parking lot 30.
